Social/Medical History If you are an adoptive parent or an adult adoptee, you have the legal right to receive all available information about the social and medical history of the birth family. You may start this process by using Marywood's Request for Service form, which will provide us with proof of your identity. Or you may call Marywood and ask that a form be sent to you. This form is required before we can release the information to you. If you are a birth parent, or if you know important information about a birth parent, particularly information of a medical nature, we strongly encourage you to communicate that information to us. We will make every reasonable effort to furnish that information to the adoptive parents or to the adoptee. Adoption Registry If you desire contact or information about a birth child, birth parent, or birth sibling, we strongly urge you to register with Marywood’s Adoption Registry. This is a process which is set forth in the Texas Family Code. If you would like to have a copy of the statutes, we will be glad to provide them. How the registry works: If you and your birth child, birth parent, or birth sibling both register, then there will be a "match." If there is a match, both parties must receive one hour of counseling before we disclose any information. This requirement is mandated by law. After counseling and carefully considering your options, you may wish to pursue further contact. The decision is yours, and we will respect your choice. We will not disclose any identifying information to the other party until we have your written permission. If you choose not to register, please help us by staying in touch so that we can keep your current address in our files. Adoption is a life long experience, and situations may arise in which we need to contact you. Searches Marywood may be able to use the information in the files to find the person with whom you desire contact. We will search on behalf of an adult adoptee for his or her birth mother.
